Registration
Registration for new students (K-8) for the 2010-11school year began in January 2010. We offer both full day and morning only kindergarten classes. Please contact Mrs. Debbie Joseph for information at 614-451-1626 ext. 155 or djoseph@cdeducation.org.
The following items are needed to complete your registration.
- Completed Registration Form - Registration Form
- Copy of child's Birth Certificate
- Copy of child's Baptismal Certificate
- Copy of child's Social Security Card
- Copy of most recent immunization record
- Ohio School Health Form - Physician's Report - Health Form Physician's
- Ohio School Health Form - Parent's Report - Health Form parent's
- $300.00 non-refundable registration fee (applied to tuition)
- If applicable, court papers regarding custody of child
- Records Release form (if entering grades 1 through 8) - Records Release Form
We will be hosting an Open House on Sunday, January 30, 2011 from 1:00pm to 3:00pm. Please join us to meet our teachers and administrators and to take a tour of our school. All are welcome!
